We realize that you are busy so we make sure to provide you the highest quality care as quickly and efficiently as possible.
What to bring with you
- A valid photo ID.
- Insurance card (if you have coverage).
- We require all patients to provide a payment method (credit/debit card) that we securely store on file to cover balances due after your insurance benefits are applied.
- Cash, check, credit or debit card for co-pay if you are using insurance or self-pay fee if you are not paying with insurance.
